Forms & Paperwork
You’ve got lodging CHECK! Registration CHECK! and your Plane Tickets CHECK! You’re ready to tackle the FORMS! You’ll find all the Necessary Paperwork listed on page 9 of the Let’s Go Booklet that you got when you took Troop Tripping- You’ll need to submit to your Activity Consultant:
Trip/Event Permission Form & Activity Approval (she should already have the one you turned in last summer when you decided to take the trip…but this one will have all the details filled in!),
Detailed Itinerary (you might not be ready to turn this in yet, but let your activity consultant know that you’re working on it. Your girls may still be planning what to do and some decisions may be based on reaching your cookie sales goals. Get it in by early April.)
She’ll also want to know that you have proof of extra insurance and have done the Safety and Risk Management Plan. (As part of your plan think about how you’ll find a missing girl, bringing pictures of each of the girls on your phone is a good idea just in case. If the girls are lodging in separate rooms think about evacuation plans during the night if there is an earthquake. What’s your plan if a girl gets injured? Find the nearest hospital and put the name and address on your Safety Management Plan)
You’ll want to have health histories for girls and adults and permission to give over the counter medications which are now in your Annual Permission and at least one Accident Report Form (I tuck mine into my first aid kit so I’ve always got one with me or the troop first aider.)
Give your in-town contact and Activity Consultant a copy of emergency contact info for girls and adults (I usually type emergency numbers out on a spreadsheet so they’re more compact. You’ll compile that list when the permission slips come back to you.)
If you’re driving you should have Transporting Girl Scouts signed on file with you and each driver should have forms for the girls in their car. All parents should have a copy of your itinerary and should know that if plans change that you’ll let them know through the In-town contact.
You’ll find all the forms on the council website. You’ll find a link to Let’s Go on the Travel Guidelines page: http://www.sdgirlscouts.org/…/online…/travel-guidelines.html It’s a good idea to read it through again, check the list 11 steps to advanced domestic trips (Let’s Go page 6) and see how you’re doing!
As always, check with your Activity Consultant to make sure that you’re using the most recent forms and procedures especially if it’s been awhile since you took Troop Tripping.
